Archives Identifier00239Date Range1909-1955DescriptionThis series consists of sets of two forms: Form E, issued under the Cremation Regulations 1928 and 1939, and a Wellington City Council Form titled Authority to Dispose of Cremated Remains. They cover from Register No. 1 to 14240. Form E provides the following information: register number, name, address, occupation, authorisation by medical referree and date, date cremated. The Wellington City Council form details how the cremated remains were disposed of and date. These forms are retained because full sets of forms do not exist for the period from 1909, when cremation began, to May 1955. After May 1955, full sets exist which include a copy of Form E.
