Archives Identifier00285Date Range1955-1992DescriptionThis series consists of a set of forms issued under the Cremation Regulations 1939 and 1973. Information provided in the set of forms includes
- the name, age, last address, occupation, gender and marital status of the deceased
- the name, address, and occupation of the applicant
- questions relating to the relationship between the applicant and the deceased
- the date and hour of death
- whether the deceased had a pacemaker or biomechanical aid
- the name of their normal general practitioner and
- who was present at the death.
Additional forms were added in August 2003
A set normally includes the following:
- Form A - Application for Cremation
- Form B - Certificate of Medical Practitioner
- Form E - Authority to Cremate and, sometimes
- Form C - Certificate after Post-Mortem Examination
At some stage [date to be confirmed] the forms changed and sets were made up of the following:
- Form F - Permission to Cremate
It may also include a form notifying a still birth, an authorisation from another area of New Zealand, or a form signed by the Coroner [where an inquest has been held].
The Application for Cremation is generally completed by the undertaker. The Certificate of Medical Practitioner is completed by either the person's GP, a hospital doctor or coroner. Information includes the time, place and location of the death, the nature of their illness, how the fact of death and identity of the person was established and conditions contributing to the death. Other information relates to discovery of any suspicious circumstances surrounding the death. The Permission to Cremate is signed by one of the Medical Referees appointed under the regulations. It identifies the person to be cremated, ensures that all legal requirements have been carried out and that the cremation may proceed.
The sets of forms are numbered with Register numbers 14241-63524, 63801.
